You'll receive a personalized review of your results, practical recommendations, and guidance on your next steps.
$497 includes both appointments and routine laboratory testing. No ongoing program commitment required.
What's Included in Your Foundations Evaluation?
1. Comprehensive Health Assessment
We begin with an in-depth review of your symptoms, medical history, lifestyle, medications, and personal health goals.
2. Personalized Laboratory Testing
Clinically appropriate routine laboratory testing helps us investigate relevant aspects of your hormone, thyroid, and metabolic health. Your initial Foundations fee includes routine labs selected for your evaluation.
3. Your AgeWell Signature Review
During a separate 45-minute appointment, we'll review your laboratory findings, discuss what they mean in the context of your symptoms, and develop an individualized plan with practical next steps.
4. A Clear Path Forward
You'll understand your results, receive personalized recommendations, and explore whether ongoing AgeWell care would be beneficial. There is no obligation to enroll in another program.
